Amticlav 625 Tablets comes in packing 10 strips of 6 tablets each. Clavulanic acid has a high affinity for and binds to certain β-lactamases that generally inactivate amoxicillin by hydrolyzing its β-lactam ring. Combining clavulanate potassium with amoxicillin extends the antibacterial spectrum of amoxicillin to include many bacteria normally resistant to amoxicillin and other penicillins and cephalosporins.
Precautions:
Prescribing amoxicillin and clavulanate potassium tablets in the absence of a proven or strongly suspected bacterial infection or a prophylactic indication is unlikely to provide benefit to the patient and increases the risk of the development of drug-resistant bacteria. While amoxicillin and clavulanate potassium tablets possess the characteristic low toxicity of the penicillin group of antibiotics, periodic assessment of organ system functions, including renal, hepatic and hematopoietic function, is advisable during prolonged therapy. A high percentage of patients with mononucleosis who receive ampicillin develop an erythematous skin rash. Thus, ampicillin-class antibiotics should not be administered to patients with mononucleosis. The possibility of superinfections with mycotic or bacterial pathogens should be kept in mind during therapy. If superinfections occur (usually involving Pseudomonas or Candida), the drug should be discontinued and/or appropriate therapy instituted.

History of allergy especially to cephalosporins, infectious mononucleosis, severe renal impairment.

Upper and lower respiratory tract infections Adult: Based on amoxicillin dose, 250-500 mg every 8 hours or 500-750 mg every 12 hours.

* Hypoparathyroid patients Ensure that serum calcium, phosphorous, and 24-hours urinary calcium are determined periodically. Predialysis patientsEnsure that serum calcium, phosphorous, alkaline phosphatase, creatinine, and intact parathyroid hormone (iPTH) are determined before starting therapy. Thereafter, serum calcium, phosphorous, alkaline phosphatase, and creatinine should be determined monthly for 6 months and then periodically thereafter. The iPTH should be determined every 3 to 4 months. Serum Calcium LevelsEnsure that serum calcium is evaluated before starting therapy, twice weekly during dosage adjustment, and then periodically thereafter, and that blood samples are taken without a tourniquet. If hypercalcemia is noted or if the serum calcium times phosphate product (Ca P) is more than 70, immediately discontinue therapy.
Special Precautions:
Idiopathic hypercalcaemia. Pediatric doses must be individualised and monitored under close medical supervision. Coronary disease, renal function impairment and arteriosclerosis, especially in the elderly.

Precautions:
1.Monitor
Response to therapy Monitor patient's response to therapy. Notify health care provider if infection does not appear to improve or worsens.
2.Adverse Reactions
Monitor patient for GI, DERM, and general body adverse reactions, and signs of superinfection. Inform health care provider if noted and significant. Immediately report severe diarrhea, diarrhea containing blood or pus, or severe abdominal cramping.
Special Precautions:
History of allergy to penicillin; severe renal impairment; pregnancy and lactation.
Dosage:
Child: 8-10 mg/kg/day in 2 divided doses. Max dose: 400 mg daily.
Renal impairment: Patients on haemodialysis: Dose should be given after each dialysis session.
Storage:
Store at 20-25°C.

Cefpodoxime is an oral third generation cephalosporin. It is active against most Gram positive and Gram negative organisms. Notable exceptions include Pseudomonas aeruginosa, Enterococcus, and Bacteroides fragilis. It is commonly used to treat acute otitis media, pharyngitis, and sinusitis. It also finds use as oral continuation therapy when intravenous cephalosporins (such as ceftriaxone) are no longer necessary for continued treatment.

Pantoprazole sodium 40mg

Pantoprazole inhibits H+/K+ ATPase pump function thereby reducing gastric acid secretion. It also has a role in the eradication of H. pylori.
Special Precautions:
Long-term therapy may lead to bacterial overgrowth in the GI tract. Hepatic impairment; monitor liver function regularly (if enzymes increase, discontinue); pregnancy; not recommended in children <18 yr; long term use may lead to atrophic gastritis.

Gastro-oesophageal reflux disease
Adult: 20-40 mg once daily in the morning for 4 wk, increased to 8 wk if necessary. Maintenance: 20-40 mg daily, increased to 40 mg each morning if symptoms return.
Hepatic impairment: Doses >40 mg daily have not been studied in hepatically-impaired patients.

Peptic ulcer
Adult: 40 mg once daily in the morning for 2-4 wk for duodenal ulceration or 4-8 wk for benign gastric ulceration.

Adult: Triple therapy: 40 mg bid combined with clarithromycin 500 mg bid and either amoxicillin 1 g bid or metronidazole 400 mg bid.

Adult: 20 mg daily.

Zollinger-Ellison syndrome
Adult: As Na salt: 80 mg once or twice daily. Up to 240 mg daily may be given in divided doses. Convert to oral therapy as soon as possible. Dose to be given as slow inj or short-term infusion over 2-15 minutes.
Hepatic impairment: May need to reduce dose.

Gastro-oesophageal reflux disease Adult: As Na salt: 40 mg daily. Convert to oral therapy as soon as possible. Dose to be given as slow inj or short-term infusion over 2-15 minutes.

Reconstitution
Reconstitute with 10 ml 0.9% sodium chloride (final concentration 4 mg/ml). Reconstituted solution may be given IV (over 2 minutes) or diluted to 100 ml in glucose 5%, 0.9% sodium chloride, or lactated Ringer's and infused over 15 min.
Incompatibility
Y site incompatibility: midazolam and zinc.
Storage:
Intravenous
IV (before reconstitution) <25 °C.
Once reconstituted, store <25 °C and use within 12 hours.
Oral
Store tablet at 15-30 °C.
Pantoprazole sodium 40mg + Domperidone 10 mg

Pantoprazole acts by selectively inhibiting the H+/K+-ATPase enzyme in the secretory canaliculus of the stimulated parietal cell. Domperidone stimulates GI activity by acting as a competitive antagonist at dopamine D2-receptors.
Special Precautions:
Lactation. Pantoprazole: Not recommended in child <12 years. Long-term therapy may lead to bacterial overgrowth in the GIT.
Domperidone: Increases serum prolactin levels resulting to galactorrhoea in females and gynaecomastia in males. Hypertensive crisis in patients with phaeochromocytoma.
